FIRST VISIT

Your initial appointment will consist of a discussion reviewing your medical history and reason for visit. A clinical exam will be performed with a focus on your specific needs and concerns and additional radiographs may be taken as necessary. We will then explain your diagnosis and treatment options in detail, taking time to answer any questions that may arise. Often, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation first and a second appointment to provide treatment.

Please assist us by bringing the following information to your appointment:​

-Referral slip and/or x-rays given to you by your dentist

-A list of current medications you are taking

-Any medical or dental insurance cards or forms

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If additional radiographs are necessary, they will be taken at our facility.

You may also save time at your appointment by printing and filling out the General Demographic Information, Health History, Privacy Policy, and Financial Policy forms that can be accessed by clicking the link below:

IMPORTANT: All patients under 18 years of age must be accompanied by a parent or guardian.
